Step 1. If you've never used DocHub before, you will need to sign in with Google (Gmail) or create an account.

Step 2. Click the "Fill a Copy" button on the top right.

Step 3. Please change document title to include camper name and click "Fill Copy."

Step 4. Agree to use electronic signatures and click "Let's do this!"

Step 5. Fill out all fields and signatures.

For signatures you can use any of the 4 provided options.

*Phone Option: allows you to use your phone as a signature pad. Scan the QR code or text yourself a link for the virtual signature pad.

Step 6. When all fields and signatures are filled out finalize the document by clicking "Finalize" on the top right.

Step 7. Confirm Finalization by clicking "Finalize Document"
